On May 13th from 12:30-4:30pm, Athletic Physicals will be completed in the Old Reserve Building.
Please download and print the attached form, it must be signed by a parent/guardian and completed prior to obtaining the physical. The cost is $25 and can be paid by cash or check made out to Mercy Health. Mercy Health will be performing the physicals.
